A 32yrs old patient came to hospital with pains in the groin region

A 35yr old patient cement worker by occupation came to hospital with complaints of pain in the abdomen  Chief complaints  The patient has complained of severe pain in his abdomen which is right loin region radiating to groin from 5days and also loss of urine. History of present illness The patient was asymptomatic 1yr ago then he developed abdominal pain and went to hospital where he got diagnosed with renal calculi and took treatment and got relieved. Then 5 days back he developed abdominal pain and went to a local hospital where he was given medication but the pain is not releived. He came to the hospital with abdominal distention and loss of urine output. He is passing loose stools History of past illness The patient did not complain of any fever and is not a known case

A 65 yr old male patient farmer by occupation with kidney problem from 2yrs has come to the hospital for dialysis. Chief complaints  The patient came to the hospital 16days back with chief complaints of decreased urine output and shortness of breath History of present illness The patient is a known case of kidney problem from 2yrs when he was taken to another hospital he has been using medication for the last 2yrs as prescribed   then he has been put on dialysis from  3months (2 times in a week).  History of past illness The patient has partial deafness since 15yrs  The patient  is also diagnosed with hypertension 2yrs back when he got to know about his kidney problem. The patient is not a known case of diabetes,asthma ,tuberculosis.  Personal history  Diet:mixed Appetite:n

A 50 yr old patient housewife by occupation  resident of narketpally came to the hospital due to reduced urine output and shortness of breath  History of present illness  The patient was asymptomatic 6months ago later she was put on dialysis for the past 5 months  1month ago she started to complain about decreased urinary output(oliguria) and shortness of breath(grade 3) The patient complained about facial puffiness which was reducing in size as the days progressed so  was put under further dialysis History of past illness Patient is a known case of  hypertension since 7months Not a known case of diabetes melitus ,asthma,tuberculosis Family history No significant family history Personal history  Diet:mixed Appetite:normal No allergies No addictions in the past Regular bowel movement Ge

A 42 year old male daily wage worker by occupation resident of narketpally came to causality with  complaints of pyrexia and presence of swellings 10 days back on 31st May.   Past history The patient is a known case of tuberculosis few months back No similar complaints in the past  Not a known case of asthma, diabetes, epilepsy, cardiac disease, hypertension Present history Diet : mixed Appetite: normal Sleep :normal No allergies Family history No significant family history General examination  Patient is concious, coherent, cooperative Presence of lymphadenopathy in his neck and abdomen region No cyanosis  No pallor No icterus  No clubbing of fingers  No poedal odema  Systemic examination Respiratory: no swellings B/L symmetrical chest  skin normal  no sinuses  no drooping of should
